NDIS Home Modification Assessment

Our registered Occupational Therapists visit your home to assess what modifications are needed to help you live safely and independently. We prepare the evidence-based reports the NDIA requires to fund home modifications through your plan.

What are NDIS home modifications — and what does an OT do?

NDIS home modifications are changes made to your home to reduce the impact of your disability and support you to live safely and independently. They range from minor modifications (grab rails, non-slip surfaces, ramps) through to major structural changes (bathroom remodels, widening doorways, stair lifts).

An Occupational Therapist assesses your home environment and your functional needs, then prepares a report that documents what modifications are required, why, and how they will improve your safety and independence. This report is the key evidence the NDIA uses to fund home modifications in your plan.

How our home modification assessment works

Referral & Intake

We confirm the referral, scope of modifications needed, your funding situation, and schedule a home visit at a time that suits you.

In-Home Assessment

Your OT visits to observe your home environment, assess your functional needs, and identify safety risks and modification requirements in context.

Get Quotes

We recommend sourcing a minimum of 2 quotes from qualified builders. You can organise these yourself or we can assist — having competitive quotes strengthens your NDIS application and ensures fair pricing.

Submit & Fund

We prepare your OT report and submit the documentation to the NDIA. Once funding is approved, your chosen builder completes the modifications.

Post-Modification Sign-Off

After the work is complete, your OT returns to conduct a final review — confirming the modifications meet the clinical specifications and that everything is safe and fit for purpose.

Frequently asked questions about home modifications

Does my NDIS plan need to already include home modification funding?
Not necessarily. If you need home modifications but don't yet have funding, an OT assessment can identify the need and generate the evidence required to request funding at your next plan review. We can help you understand this process and prepare the documentation needed.
Can I get home modifications if I rent?
It depends on the tenancy agreement and the type of modification. Minor modifications are generally easier to approve in a rental property. Your OT can advise on what's typically achievable, and we can document the clinical need regardless of property tenure — which is the critical first step.
Do I need to use a specific builder?
No. You're free to source your own builder. We recommend getting a minimum of 2 quotes so you have competitive pricing. Our report provides specifications that any qualified builder can follow, and we can liaise directly with your chosen builder during implementation if helpful.
How long does the assessment take?
An in-home assessment typically takes 1–2 hours. Complex assessments involving multiple modifications or a larger property may take longer. The written report is typically delivered within 5 business days of the assessment visit.
My home is rented — can modifications still be funded?
The NDIA considers home modifications for rental properties on a case-by-case basis. Minor modifications are more readily approved. Your OT will document the clinical need clearly, which is the critical first step regardless of property tenure.
What happens after the modifications are done?
Once the builder completes the work, your OT will return to conduct a post-modification review. We check that the modifications were installed to the agreed specifications, that everything is safe, and that they're meeting your functional needs. If anything needs adjustment, we'll document that too.
